Submit REMS opens the portal, finds the patient (even with nicknames or misspellings), checks for a duplicate on that date, fills the whole form and confirms it's complete. You review and press Submit.

No. It fills and verifies every required field; a member of your staff reviews and presses Submit on the REMS portal. The attestation stays with your practice.
No. The extension has no server connection for patient data. It reads the note on your screen and types into the REMS page you're logged into. Read the privacy policy for exactly what is stored on your computer and the BAA we'll sign if you need one.
